CRNE mock question

See if you can answer the following CRNE style question correctly:

1. The nurse is reviewing a client's nursing admission notes from the emergency room, where the client spent 3 days. The nurse sees an entry in the paper chart stating "patient is manipulative, and lies". What is the appropriate action of the nurse in this situation?

     1. Delete the entry by drawing a line through it and initialing it
     2. Approach the patient regarding your concerns
     3. Assess the patient to determine if he is manipulative
     4. Ensure that your charting is objective and factual

Rationale: When charting ensure that your statement remain objective and factual. Avoid using language that is judgemental. Nurse's never delete another professionals written note.

The 2011 Canadian Registered Nurse Exam (CRNE) takes place:

According to the CNA in order to register for the exam, you must go through the process according to the provinical regulatory body that governs your area. Also, please check on the CNA website to see when the application deadline is and where the closest CRNE testing centre is.
